The table itself (border, columns width, background color, total width in pixels or in %) you will define in the head of the table, right after which you will paste our PHP-code.
You may take the code of table's head form this example. Take our file affil.css with all styles and add it to yours. Close table with </table> tag right after the last credit cards code.

To make your table's borders visible add to your css-file styles for left, right and all other columns:

TD.column_mid { BORDER-left: gray 1px solid; BORDER-bottom: gray 1px solid; font: verdana; font-size:10px;} - middle columns
